NS Health Expanding App to Prepare and Support Heart Surgery Patients.

Nova Scotia Health is expanding the use of the digital app, SeamlessMD, to prepare patients for heart (cardiac) surgery and support them throughout their recovery. 

Launched in 2023, the SeamlessMD Digital Care Journey app offers remote monitoring of recovery, allows patients to report their outcomes, and provides care plans, evidence-based education, and reminders. The app has been in use for patients undergoing gynecological cancer surgery, colorectal surgery, and bariatric surgery.

Health Information Scientist, Ryan Gainer, who has been working on the implementation of SeamlessMD for cardiac surgery, said, “SeamlessMD is part of our efforts to ensure patients are receiving treatments that are right for them and have the best possible outcomes from surgery when it is needed.” 

The SeamlessMD tool helps give patients important information they need to prepare for their surgery and recovery and stay connected with their care team. 

The implementation of SeamlessMD was funded by Nova Scotia Health’s Innovation Hub and has been shown to improve patient outcomes, reduce anxiety, decrease the number of patients having to visit emergency department visits or be readmitted to hospital, and allows patients to return home earlier which frees up hospital beds to care for other patients.
